Dear ____:
I am writing to inform you that a University computer located in the College of Arts and Sciences containing personally identifiable information was stolen from an office on February 21, 2009. The type of information on the stolen computer was directory information (name, address, phone number), Rocket number, college, high school GPA, and current GPA. Although, we have no information at this time that any personal information has been accessed or misused, the University has an obligation to contact you in circumstances such as these.
A police report was filed with The University of Toledo Police Department and an investigation is ongoing. The University of Toledo's Information Technology Department contacted Dell Inc. to report the stolen computer and asked to be informed should Dell be apprised of the computer's location. In addition, a tracer has been placed on the computer should the person who stole the computer attempt to log on to the University's network. The Compliance Privacy Office, in conjunction with the police and Information Technology Department, worked to determine what information was stored on the computer's hard drive.
The Compliance and Privacy Officer, law enforcement and Information Technology departments are working together to determine any potential risk to your personal information. Although the computer was password protected, personal information about our students was on the computer and some risk exists that others may gain access to it. However, at this time, we have no reason to believe that any personal information has been accessed.
For more information about identity theft, how you can protect yourself, and how to check your credit report, please see our Information Technology Web site at: http://www.utoledo.edu/depts/it/Security/Awareness/Identity_Theft.html. Also review the Federal Trade Commission's Web Booklet on identity theft at: http://www.ftc.gov/bcp/edu/pubs/consumer/idtheft/idt04.pdf.
Again, we have no information at this time that any personal information has been accessed or misused. We apologize for any inconvenience. Should you have any questions, please e-mail me at officeofcompliance@utoledo.edu.
Sincerely,
Lynn Hutt
